Through our long-standing partnership with the University of Michigan Health System’s Department of Physical Medicine and Rehabilitation, we are encouraging all to take part in a community-wide survey of individuals with disabilities.

The survey is seeking information about health care practitioner experiences.  All information is kept confidential.

You will have an opportunity to win a $100 gift card for completing the survey. As well, we are looking for participants for a focus group to discuss similar topics. Each participant in the focus group will be paid $50 for their time.

Please see the letter below from Dr. Sonya Miller from the University of Michigan.

We are asking Ann Arbor Center for Independent Living consumers to complete The Attitudes Towards Disabled Persons survey, which is part of the project Improving the Health of Persons with Disabilities through Medical Education (IRBMED HUM00014358).  The survey you are about to take is part of a larger study in which the long term goals are to establish a better understanding of care needs among existing patients with a disability.  Your individual answers to the survey will be kept anonymous.  It takes about 15 minutes to complete, is a reliable and validated research tool and you could win a $100 Visa gift card.*

Please go to to review the consent form and begin the survey.

Your opinion is very important to us and your participation would be greatly appreciated.

Thank you.
